Medical note for work, help.

I needed a medical note to be off for 2 weeks from work. Dr. Fielding's secretary wrote me one but it's on his medication prescription paper, is that a valid medical note to be off from work? Also, the paper had his name on it, along with "plastic and reconstructive surgery", his address and number. I kind of wanted to be discrete about it from work... anyone got any tips?

I work part-time, do i get 2 weeks pay of my usual salary?

Go see your GP for a note dude... I'm sure he/she will oblige! 

As for getting paid while you're off... I highly doubt your employer will pay you for being off... unless you get sick days...
