puffy nipples for friggen ages!!!

hey all, I'm 18 yrs old and I've also had puffy nipples for what seems like forever, i reckon i was about 12-13 when i first got them, I'm a thin bloke and do stacks of exercise close to about 30km of running every weekend, i am  boundary umpire for afl football, i am sick of these friggen man breasts but everything i do doesn't change a thing, I'm sick to death of em and want them gone. it really embarrassing and makes its hard to get intimate with women, when i first noticed them, there were lumps under both nipples, like a fruit tingle, then gradually the lumps got smaller and now they are really Small, but foe some reason  still have puffy nipples?, i want to know whether I'm going to need surgery or if its just hormones, any suggestions or advice would be most appreciated??
