Yes, puffy nipples at a minimum.  And perhaps some mild gyne -- which would have to be verified by a surgeon.

Best advice is to seek consultation with a gyne specialist in your area.

What kind of surgery?  More than likely lipo plus excision, which is fairly standard for most gyne procedures.  However, you should speak to a gyne specialist who can examine you and make specific recommendations for you.
