Can muscle tissue be excise accidentally?

Could muscle tissue be excise accidentally in an operation? or the gland tissue and fat tissue is way different than the muscle tissue and it would be hard to confuse it? Also can the fat tissue be remove via excision or only in liposuction? Like you are cutting the gland and cut off a fat tissue aswell?
Thanks for any response, had my surgery 1 year ago didnt go as planned /: currently dealing with the "mental scars" and have a lot of questions in my head.
