You don't have to have fascia damage with muscle exposure to have adhesions. And dermis can certainly scar down to underlying muscle OR fascia. You would be less likely to have adhesions if there is subcutaneous fat present. But keep in mind that a crater deformity doesn't have to have adhesions or tissue stuck to anything below it. Just the absence of sufficient tissue (even fat) for support will create a concavity under the nipple areola complex, or "crater". Hope this answers your question.
