Breasts growing, not nipples

Is there a difference between breasts growing (i.e. more fat, and rounded) and "puffy nipples" i.e. glands, and slightly pointy?

Is it all estrogen?

Grrr - why won't my endocrinologist put me on testosterone, and something to lower my estrogen?

Also, why, when 98% of testosterone produced by men is inactive due to it being bound by SHBG, don't people take drugs to lower their SHBG, thus increasing their free Testosterone levels rather than putting more testosterone into themselves?

I hear that broccoli and cauliflower can help lower SHBG levels - are there any other does/don'ts for this?
