nipples

has anyone ever had nipples that stick out post up but when you massage them or press them in hard, they look normal?  Mine do this and when i lie down with my arm over my head and massage they even indent..what could that be from and why isnt it always flat

It sounds to me like the plastic surgeon removed gland tissue from under you nipple.  Now there isn't anything under it.  What kind of surgery did you have?

i had both lipo and excision...the outside of my chest looks better but the inside (the nipple and around still seem puffy)..the best i can explain it is , its like pre-op but there is no gland inside.  i dont get why when  i massage the nipple gets flat for a little while

Massaging i guess stimulates the nipple and makes it go smaller, same reason why pinching it makes it go smaller.
