Puffy nipples have got to go

Alright hey. I have got a story and some questions. First off story. I believe that I've had gyne for 3 years starting when I was about 12. I had little lumps inside of the nipple but slowly they sunk and left me with two puffy nipples that show threw my shirt. I have two pe classes and each are terrible with this problem. I was wondering if it is at all possible for me to get surgery during the summer to remove this gyne and have it go away permanently, by the way I live in Wyoming and (of course) the nearest surgeon is in denver. Please help. Thanks
