I have had gynecomastia surgery 4 months ago. My doctor performed only lypo he told lypo would suffice because my case was minor. Now i have doubts. My nipples still look little bit puffy. I am confused sometimes my chest looks normal sometimes it does not. can anybody tell me what should i do to decide if i still have gyno or not.

The photos you posted don't clearly show puffiness of the nipples still, but that's often difficult to demonstrate if the room is cool and so on.  You still have some fullness on both sides, however, so at the least, you may need some additional liposuction to get a flatter contour.  If the nipples are still puffy, it would make sense to reduce that tissue through a direct incision adjacent to the areola.

It looks like there is still some residual tissue in the whole breast area. You need a good exam to tell for sure. Good luck.
