Probably a real dumb question,but when the Doc is doing the surgery, how do they determine what is fat, muscle, or gland. Couldn't they be removing part of the muscle when they are getting rid of the bad stuff?

Not if the surgeon knows what he is doing!!  Removing (or even entering) the chest muscle is a real no-no, and is probably the cause of some hematomas after surgery.  That is why is so very important to go to a plastic surgeon who is sufficiently interested and experienced in gyne surgery.  Do your homework!!
