Gynecomastia comes in many shapes and sizes, and the best operation is tailored to the specific presentation.  In most cases, this involves liposuction along with direct excision of tissue (gland) under the nipple, but without an understanding of your history and an examination of your case specifically, your question is too general to answer with any specificity.  The best thing to do is to see a plastic surgeon, preferably someone who takes care of gynecomastia on a regular basis.  And yes, the results are long lasting.
