I had lumps since the surgery but over the last month my right nipple has developed new lumps and is very tender/sore to touch. It looks similar to how my original gyno looked in fact.

Going to see my surgeon next month.

I am not sure whether this is new scar tissue (if that is even possible). Anyone had experience of this?

This is most likely scar tissue.  It is very unusual for breast tissue to re-grow. Almost all tissue is removed in surgery, with exception of a small amount for nipple-areola support.  And scar tissue can be treated with injection of a steroid chemical, usually Triamcinolone (Kenalog). Good to hear you're following up with your surgeon.
