when the gland is removed does your aerola just instantly go flat? or is it caused by using the compression vest?

Well, they say that skin is amazingly elastic, and so, to answer your question, yes, once the gland is no longer there to push out your nipples, they should become much flatter.

In my case, I have so much gland / fat that my nipples don't potrude, but I have developed what I would say must be a B cup, so I have full breasts instead of puffy nipples.  When I clamp down on them I can feel inside large chunks of glandular mass.  But anyway, yeah, from what I can tell having the gland removed should make your nipples go flatter.

Your nipples will go flatter once the gland is gone, but may not go instantly flat. There may be some loose skin which needs to have time ot tighten out. This is what is happening with me atm, and it's getting better all the time.
