trying to get a referal to see an endo, help

i get my physical in 2 weeks, i need to see an endo to see if my gyne is coming back (which i dont think it is and its just weight gain, 10 ibs in a month due to lifting)

(never went to an endo before hand)

what do i ask the primary doc, what can i expect her to say/do

my father has a ppo whcih i am covered due to im still in school, how much am i looking to pay assuming insurance pays for it ?
