A testicular taution, if not treated quickly, can cause the testicle to die. The twist in the veins and arteries supplying it with blood are blocked.

Anything that impares blood flow to a testicle can cause it to produce less testosterone, or even stop all together.

A testicular torsion however would be so painful, that nobody would leave it untreated for long. As for any lasting damage after the testicle is un-twisted, who knows. I'm sure it's possible. Your friend obviously didn't have any lasting effects.
