Tumescent technique doubts

Hello doctor,

Is it possible to remove glands by menas of Tumescent technique...?

Personally im planning for a revision surgery under this doctor so will it be fruitful..?

Please advice me...

The tumescent technique is used by virtually all doctors in performing lipo, including lipo for gyne.  It may work well for small areas of fat only -- but it cannot fully anesthetize the breast for gyne-type lipo.

Your case, in particular, is a revision -- which means that you have scar tissue present.  Scar tissue acts to prevent the normal spread of tumescent fluid and local anesthesia fluid.

If I were you, I would not opt for tumescent alone -- much too painful.

Let me clarify what is meant by tumescent technique.  Tumescent technique is just a means by which the tissues are anesthetized and prepared for surgery -- it is not the surgery itself!  One must still either perform lipo or excision or both.

Speaking in generalities, when you have already had surgery, the body has healed with scar tissue.  And that scar tissue makes further surgery, and further anesthesia more difficult because the anesthetic solution cannot percolate around the tissues easily due to the presence of scar tissue.

I have performed minor revisions under local anesthetic but much prefer intra-venous sedation for any surgery, be it original or revision.  Other doctors prefer general anesthesia.  It is a matter of the surgeon's preference and also his assessment of the patient's potential pain tolerance.  Local anesthesia will require painful injections until the tissues become numb.  Intravenous sedation or general anesthesia are totally pain free.

I would suggest you discuss the anesthetic options with your surgeon prior to your revision surgery.

I agree with Dr. Jacobs, this is surgery best done with Sedation or General in addition to the Tumescent Technique Anesthesia for Gynecomastia. The Tumescence alone is just too brutal without the added assistance.  I prefer sedation for my patients' Comfort with Gynecomastia Surgery.  Cutting corners is just not a good idea.

This surgery and Anesthesia for Plastic Surgery are art forms.  Just because a doctor uses that method does not mean the results will be the same.  That is why checking actual before and after pictures, patient experiences, and other factors about the specific doctor are so critical.
