I'm about two stone overweight - have lost 3 stone already this year. Planning the op in London for November time when I should be in and around my ideal weight (hopefully!).

Would really prefer to have lipo only as I don't have puffy nipples, I guess the surgeon would be the best person to ask here I guess?

Youve got a moderate case. What surgeon you planning to have it done with? Dont bother posting in the ask the surgeon section you have it! Im not sure if its bad enough to warrant skin removal i think you might be ok with lipo and excision.  Good luck with the weight loss

From what the surgeons on this site have said, you will require both lipo and some gland removal if you want to take care of the problem satisfactorily.

Rate you?? What is this, some sort of f-ed up competion?

Your a 10, or a 1, or a 5...whatever you want.

The point is, how badly does it bother you and do you want the surgery. If so, you will require glandular excision and lipo.

I agree with the comments about rating... not much point really.

You have moobs and they are pretty significant IMO.  You were very overweight, have lost weight and are still losing, so well done on that. 

  I think you really need a consult with a good surgeon to assess the situation - because to my (completely untrained) eye it looks like you have stretch marks and may possibly have an excess skin problem as well as gyne.   Lipo might do the trick but you may not ever get a perfectly sculpted chest, so realistic expectations are important. 

Keep going with the weight loss and make sure you find a surgeon who has experience with similar cases. - Ask to see before and after pictures.

Carefully pick your surgeon!

You have a lot of excess skin, and if you have standard lipo/gland removal surgery without any skin removal, you may be unsatisfied with your result because the skin will hang some - the old-man boob look.

You need to find a surgeon with a lot of gyne surgery experience, particularly with overweight and older men. He/she can advise you on realistic results for different types of procedures (and costs).

Rate me makes little sense in gynecomastia or female like breasts in men. It is like trying to rate a female breast.

Congratulations on your weight loss!  Does it not feel much better with that weight off?  3 stone or 42 pounds is a significant amount of weight loss! But 2 additional stone or 28 pounds is not trivial.

Weight Loss Can help with the fat of gynecomastia, but not the gland. Remaining fat, gland, and sometimes the skin can be residual contour concerns.

From the image shown, nipples are low but how low is not easily assessed. Standard Pictures to Show Gynecomastia and Loose Skin Issues After Weight Loss can provide additional detials. Some are able to lose weight and not have a loose skin component.  This is not a black and white defined issue, but one of grays and subtleties.

Weight loss before surgery is usually much better than weight loss after surgery. Weight loss is a coarse tool, Plastic Surgery is better reserved for refinement. This is especially true when tissue sagging is a factor.  Why lift sagging tissue, lose more weight, and see that tissue sag again from further deflation? 

Weight loss and surgical sculpture is a series of compromises. What suites any one individual will vary.  Ideal sequence that I recommend my patients:

  • Get to a weight you are comfortable living with.
  • Let the skin adjust as much as it will. It can take from 6 to 18 months for skin equilibration after a gastric bypass and major weight loss.
  • After Massive Weight Loss, Consider Tightening Lower Tissues First. There is little sense to lift the chest and then have a tummy tuck, lower the chest result, requiring a revision chest lift.
  • Then Address the Chest.
  • Use No Surgery Body Shaping Garments as emotional support not to rush the process.

Bouncing redundant flesh also can be a major emotional factor stressing any patient trying to lose weight.  That is why I posted Videos of Compression Garments dealing with gynecomastia and stabilization.

The remaining fat and sagging can be stressful for a work in progress, being patient, finishing off the weight loss first is a better option.

When it comes time to look into surgery, male chest sculpture with loose skin is an art form needing special skills. Check for examples of the surgeon's work in this variation before risking a surgical gamble. Bad surgery with this problem can sometimes be impossible to fix.

Anyway, I saw a specialist yesterday who is reknown for micro/vaser lipo. He said a few things that contradicts a lot I have read here;

- Lipo can remove all/most of the gland
- The compression vest will only need to be worn for 3 or 4 days. He said all they do is squeeze out remaining fluid left left from the chest area and do not actually help compress the chest to make it firmer

Reckons he can do a good job for me, but are his views correct?

If someone can back up such claims with images including multiple angles, arms down, arms up over head, and flexing muscles such as my Standard Photographs for Revision Gynecomastia or Standard Videos After Gynecomastia Surgery, then perhaps I might believe. To date such I have yet to see such proof anywhere. However, I see just too many patients unhappy after such claims were made. I recently reviewed over 600 patients unhappy after other surgeon's techniques over the past few years. The largest segment was from those who were told by their surgeons that liposuction alone will manage the problem.

For gland, liposuction does not remove gland, it digs a hole in the fat around the gland as you can see in these Puffy Nipple Complications of Gynecomastia Surgery. Firm gland is easily removed through a small incision with good sharp scissors. Softer gland cuts easily but is harder to differentiate by feel from the surrounding fat. This makes the job of targeting gland first more difficult.

Leaving this gland behind would not be a problem if it did not affect the contour on animation. That is why judging results while living life such as playing volleyball, football, swimming all with the chest exposed is the final answer as to if a technique works. Be wary of a doctor's claim without such documentation. Look to see how the results move!  If there is a contour problem after surgery, you will be like the hundreds of others I have seen in the past angry after spending money on other surgeons and still seeking a decent result.
