Oh, and an afterthought. Picking your surgeon should involve one who you trust in judgment of when to operate, how they sculpt, how the result moves, the path each patient needs to take to get to the final result, the education they offer their patients, and passion for this problem.  That is all part of the skill of this sculpture.
