how much is fat and how much is gyneco (if any)

The distinction is rather meaningless. Either way the answer would be surgery.

No matter how much weight you may lose, nothing will reconstruct your chest, tighten the skin, raise the nipples, etc, other than surgery.

Suggest you get down to your lowest possible comfortable and stable weight -- and then consult with a plastic surgeon who is experienced in gyne or in chest reconstruction after significant weight loss.

Without an examination it is not possible to give a precise number as to the percentage of glandular vs fat in your breasts. Based on your photo you have a significant gynecomastia with excess skin. The skin will have to be addressed to optimize the result in your case.
