Lost 100 pounds and ready for surgery (PIC)

I am planning on having surgery in the next 2 months.

Could any doctor tell me if I require free nipple grafting and skin reduction which will result in those severe scars or will 'normal' surgery be good enough?

Just good old diet and exercise. Nothing magical. Still want to lose about 30 pounds though, but that will be achieved in a few months
