Gynecomastia or just Moobs

i am 19 years old and i weigh around 240.  I do workout on a daily basis.  I'm not saying I am completely obese but I can lift a good amount of weight.   It just my breasts seems to change in size during random times.  Sometimes, my breasts will be normal, and sometimes, it sticks out. Then sometimes, its one or the other.  Just wandering if surgery is necessary or not.  Could be a huge help if someone can help me with some answers.
