Gland and scar tissue

I have a hard mass under my right nipple remaining after surgery. It is about the size of the tip of my finger.

Would you doc's be able to tell the difference between scar tissue, and a gland that was not fully removed - during a physical exam?

Related question - would a glad not full removed - lead to gyne returning?

The small remnant of tissue that we leave under the areola can sometimes be felt after surgery, particularly in the early months.  Over time, it may soften or shrink if it's mostly scar tissue.  In the long term, that tissue is sensitive, and if stimulated, it could re-grow.

The lump maybe left over from surgery 20 years ago. I had the surgery again 8 months ago. I am having a revision surgery on my right side this winter. I would like that hard lump removed completely like my left during the revision. I worry that that lump is a gland that is causing my right side to continue to grow. Like a little factory or something. His method of using a tiny tube cutting cannula is challenging - with a little rock like I have in my right side.
