Will surgery cure both puffy and pointy nips?

Will surgery completely flatten the chest out? Because right now my nipples are puffy when it's warm and very point/wrinkled when cold. Will they become less pointy?

Yes.

When properly performed, gyne surgery will reduce the puffy and pointy nipples.  The result should be a nipple and areola which is flat and flush with the surrounding skin of the chest.

Often overlooked is the fact that puffy nipples rarely exist alone -- there is frequently excess breast tissue as well.  This, too, should be removed at the same time.
