Usually the use of kenalog is restricted to a small amount of what (by history) seems to be scar tissue.  In other cases, where there seems to be a lot of excess tissue, and it cannot be ascertained whether there was tissue left over from the initial surgery, then surgery is usually indicated.  This is where you need the expertise of an experience gyne surgeon who does a fair amount of revision surgery in order to evaluate what is needed to be done.
