Hello doctors! My op went really well and my scars are not easy to notice at all! There is just one little thing and that is that my nipples are still a bit "puffy"... My doctor told me that he left a small piece of the tissue to make the nipple not "sink into the body". Will they always be puffy still or is there a chance that they go "normal" and is there something you could do?

Only time will tell.  Give it six or more months -- and then if the nipples are still puffy, you may need a revision procedure.  Kenalog will not work on residual breast tissue.  Return to your surgeon to discuss.

It's not uncommon for the sub-areolar remnant of tissue to swell after surgery and stay swollen for months.  I have had a few patients who were still a little puffy at 6 months, and I've injected Kenalog with improvement, suggesting that the puffiness was scar tissue or swelling.  If it doesn't resolve with time or Kenalog, then further excision might be required.
