I've had this since I was around 12 years old. Its really starting to take a toll on me psychologically as of late. It affects me in more ways than I can describe. I'm fairly certain its gyne/puffy nipples but I just want second opinions. Any thoughts as to the severity of my case?

By looking at these pictures I cannot tell how severe is your condition, but yes, it is Gynaecomastia.

OK, Yes, but not all that bad.

Have you been shaving your torso?  That tends to give a more feminine appearance than the small breasts do in my opinion.
