Hi,

I have to fix a second appointment with a surgeon... what kind of questions can I ask to have a full overview (I've a severe case of gynecomastia with gland hypertrophy and a lot of fat and excessive skin and quite thin / pale skin type).

Any suggestion would be helpful!

I think the critical issue and questions are related to the surgeon's experience. We as physicians refer to the practice of medicine in general and surgery in particular.With more practice  you get more experience.With more experience you know what works and what does not work and therefore get more consistent and better results.I suspect you can get a feel for the amount of gynecomastia a surgeon does in numerous ways.

My main issue is that my skin is really really thin and pale (so that you can see the veins in particular in chest area and also stretch marks on the side of the breasts) so the scars are a major problem and results can be poor, painful...

You are getting waaaay ahead of yourself.

If indeed your problem is severe, then more aggressive surgery would be needed to remove excessive tissue, tighten skin and re-position a low-lying nipple.  Pale skin and prominent veins do not predict bad scars. 

You will have to reach a decision sometime about the procedure and understand that the scars will be a trade-off for a better, trimmer, tighter and more comfortable shape.  You will be able to fit better in clothing, participate in sports, etc.  And the scars will fade in time.  Ultimately, though, it is your decision to live with it or not.

Any pain that you fear will usually be confined to the peri-operative period.  In my experience, most guys have minimal pain from these procedures.
