Hi i'm 21 years old and i have problem with puffy nipples, so let me explain the situation..

Through puberty i had variable weigh, from skinny to a litle overweigh (nothing extreme) but i never had problem with puffy nipples until on 21 years i started going in gym. After i started in gym i seen changes in my chest and nipple, chest was bigger which is ok knowing that i added some muscle but my nipples got puffier for first time in my life so after 3 months spent in gym i decided to quit - because with extra muscles on my chest nipples was even worse.  I never used steroids or something like that while i was going in gym, just whey protein (no soy protein) and l carnitine

BUT

Interersting thing is that i don't have puffy nipples whole time, example when i wake up they are little puffier but after 5 minutes they are ok and they remain same (hard) until evening is coming - after that from time to time its gettting more and more puffier. Also i see that after hard physical activity (sport, work) they go puffy or after i eat junk food, soda and other "bad" food like pizza, pasta.

Sure this affect on my confidence, it looks awful through shirt so i'm now in diet to decrease my body fat levels. But i'm not sure that will help, so my question is how this look docs ?
Is it gynecomastia or lipomastia or maybe something else ? is there any chance to i fix this on natural way or i must to go on surgery to fix this problem  ?

Nobody got experience with situation like this ?

I'm losing weigh but i don't see too much improvment, still puffy nipples appear how day is going expecially if weather is hot :/

Based on the photographs, you have mild gynecomastia (It's difficult to say with certainty without examining you).  In mild cases, oftentimes the puffy nipples are the only presenting complaint, and they will come and go based on how warm you are and so on. If this is something that bothers you a lot, you should at least be evaluated to determine if you have gynecomastia to account for the changes you're experiencing.  If you have it, then you need to decide if you want to have the surgery to address it.  Weight loss and exercise will help to some extent, but true gynecomastia will not go away just with exercise and diet.

Thank you doctor for the answer. How i already said i'm cutting bodyfat till the end of this month so we will see the results after that, also i reduced sodium and gluten foods so there is slighty improving to my condition i think. 

And i was to local doctor (not specialist for this type of condition) which said that there is no gland behind the nipple

It looks like mild gynecomastia. Later in the day you are probably more relaxed so the nipple also relaxes and looks more puffy. I doubt that weight loss will help. You should see a surgeon who does a lot of gynecomastia surgery. Other docs might not really appreciate the condition as much. Good luck.
