The appearance of my chest  has become little better with muscle grown around it .. But Not completely satisfactory.. It has always bothered me as having an attractive personality but bad chest.. I have spoken to my parents about it when I was young.. My parents are completely against the surgery.. And even after many attempt I have failed to convince them.. (They being uneducated about this ) anyways now being old enough and self depended.. I am ready to go ahead with the require procedure.. But only need few help and advice to what should be done to my problem.. And I live in India.. Can also advice me with the surgeon and the best what should be done for it.. Please find my attached picture and do let me know what should be done..

I am 174cm tall and weight 74kgs

I grew up in a time when this kind of surgery was really rather crude compared to the procedures done now.  That could be the frame of reference your parents were working from.  In that time, results were not nearly as good as what a skilled Cosmetic Surgeon can do now.
Yes, you have it.  In spite of your claim to have your diet under control, you do appear to be somewhat overweight, that would contribute to, but not be the entire problem.

It certainly looks to me that you have gynecomastia.  From your history you have adolescent gynecomastia the most common form of male breast enlargement.  Gynecomastia surgery will provide a nice looking chest. You are young and your skin will contract or shrink back very well. You need to see someone with experience with performing this surgery.  Our panel of surgeons on this website are experts.

You have gynecomastia. I have a group of patients who are in the military and more often than not have the surgery under local. These individuals are navy seals ,army rangers, marines to name a few. Based on your photos you are not in the aforementioned group. There are a number of options available for sedation during the procedure. You should discuss the options with your surgeon.

Agree with Dr. Pensler.  You do have gynecomastia and surgery under general anesthesia (or twilight sleep) would be preferable to local anesthesia alone.

Do you research and find a gyne expert surgeon in your area or consider traveling for such expertise -- it will be worth your while.
