Result at 3 months

I'm not sure if im being unrealistic but i feel like the doctor was a too conservative on the removal of both fat and gland. His comment was that what is causing the appearance is loose skin. I will say if i pull up on the top part of my chest or tense my neck up the appearance flattens out, lending credence to his opinion. 
I'm still overweight but figured i would get a flatter look. If i just drop weight will it resolve? probably can't tell that from the pictures. 
there was a lot of gland removed (don't have those pictures at the moment) but there is still some feminine look from the 45 degree angles. Very noticeable difference from the before and after with a shirt on. Posture has improved dramatically due to not hunching over anymore.

As a gynecomastia surgeon, I would not be happy with the outcome. However, this is easily corrected by further excision.  Weight loss will also reduce the area but probably not as much as you would like.  I do sometimes leave more tissue if the skin is of poor quality. This may be the case.  Good luck

Ok, thanks Doc. since my doctor thinks the result is good, what would you do? just find another surgeon if he refuses to make any revisions?

Your surgeon exercised caution in removing the excess tissue. That was appropriate. You appear to be overweight and to be a middle aged guy.  Those are two problems that mere removal of excess tissue under the skin cannot address.  The skin has stretched out and has lost much of the needed elasticity.

IMHO, you have some residual excess tissue which could be removed but it  would also require some type of skin removal in order to achieve a better, tighter chest.  Skin removal would leave scars -- but the good news is that mature men tend to heal with much better scars than younger men.

Strongly suggest you seek a second opinion from an experienced gyne surgeon.

You would benefit from an additional surgery as you suggest in your post.There are a number of things that can be done to optimize your result based on the limited photos.

I think that your result can be improved but you will need a peri-areolar skin resection. They scar should heal well and it will give you a flatter look. Good luck.
