I had gynecomastia surgery with skin excision more than three months ago. One side is still uncomfortable, lacks a full range of motion, and feels like a "lump" extends across the scar. It causes enough discomfort and pain that I can't, say, go for a jog - the "jostling" is too much. Again, this is more than three months post surgery. There has been zero improvement over the past month. At what point should I start getting really worried?

This would be an excellent time to return to your surgeon with questions and a discussion.  Only he  knows what was done and he would be the best person to provide you with answers.

Your case is atypical so without knowing what was done it is not possible to accurately comment. Having said that  with skin excision it takes more time to mature completely, average would be around 6 months.
