Question for you:  if local anesthesia was the best method to do gynecomastia surgery, why don't many other doctors use it?  The answer is that there is pain when this is done -- I have heard that from many patients who have come to me for revision surgery after having had surgery in this manner.

Local is not a good option, in general, for primary male breast reduction surgery.  The issue is pain, bleeding, and unable to perform at the highest level through a very small incision. Nearly, 50% of my gynecomastia practice is revision surgery and a significant amount of these patients have had local anesthesia.   Safety and good results the first time are what patients want.

That is the opinion of dr lebowitz, which apparently is the best gyno surgeon in the world.

"Best in the world " is an accolade which should be earned by one's peers -- and not by self description.

I can probably count on one hand the times that I have performed gynecomastia correction using just local. I use a lot of tumescent fluid during the surgery and it would be uncomfortable for the patient when I'm putting it in if they weren't also receiving sedation. I also believe that the entire procedure should be an "enjoyable" procedure which happens when using IV sedation. I don't think the method of anesthesia should be a higher priority than choosing a surgeon who gets consistently good results with a low revision rate.

In what situations will you perform gyne under local?

I almost never perform correction under just a local. It would have to be a very small amount of breast tissue directly beneath the nipple. Most guys have more than this even though they don't realize it.
