I have very palpable hard tissue behind left nipple and much less so on the right.

Sometimes I can feel it on the right side, other times I can't.

In a recent consultation, the doctor (plastic surgeon) suggested only doing the procedure on the left side but I've read other places online saying if you're going to get one done during surgery, get them both done..

Thoughts?

I made an error many years ago by trying to reduce the larger side to match the smaller side.  Big mistake.  Very difficult to do. The patient returned and wanted the un-operated side treated because of the resultant size discrepancy.   Best advice is to work on both sides and make each of them as good and normal as possible.
