insurance coverage

I have afifteen year old with gynecomastia to the extent that anyone has it in any of the pictures on line. After taking him to the pediatrician, endocrinologistx2, plastic surgeon and psychiatrist, the insurance company has turned me down x3. I am continuing to appeal but does anyone have any arguments or suggestions that would help convince them. I just got through statine my case inperson to a panela nd am awaiting their response. I would appreciate those who have been where I am to respond
