Puffy nipples/gyno/or just fat?

28 yrs old, 160 lbs, about 17% bf. I've always had puffy nipples etc. 
I am 30 lbs lighter than I was last year, but still have flabby "breasts". 
Should I go see a doctor?

Your photos reveal excess tissue on your chest, which therefore qualifies to be termed gynecomastia.

Best to seek a thorough evaluation by a gyne expert in your area -- or consider traveling to consult with one.  Your questions should be answered in that way.
