A little about me. I'm 27 and have had gynecomastia from the age of 13 or so. I'm 6'2, 195lbs, and in pretty good shape. I rarely take my shirt off in public, with the exception being in the locker room at the gym; however, it is only after I have pinched my nipples to make them hard enough to be less noticeable. Hoping to have surgery in the fall with Dr. Jacobs. Can't wait to enjoy the breeze with, or without, a shirt on!!

The photos, while not perfect, do reveal your problem -- puffy nipples and perhaps some additional surrounding extra tissue.  Assuming this has been stable for roughly 14 years, then you can reasonably assume it will not spontaneously disappear within the near future.  Surgery would be helpful to remove the excess tissue and flatten the puffy nipples.
