Question at pre op after post op :D

hello, i had my op 4 months ago, but ive gota question now!

heres the pic before



as you can see,the gland is (always) directly unter the areola,under the nipple.
but WHAT causes the whole breast to enlarge,and not only the nipple then like puffy nipple?
