i dont have a lot of fat just puffy nipples and want to know what kind of surgery i need to get done i feel a small lump under both of them would removing that flatten them or is it just because of lose skin.

You can find many examples of Surgery for Puffy Nipples on this link.  Typically skin shrinks very well for my patients such that skin reduction is not necessary.  The deformity is almost always a combination of gland and fat as you can see in the Puffy Nipple Anatomy.

You are young and not overweight.  You do have puffy nipples, which represents excess breast tissue directly beneath the areola.

You would do well with a simple excision of the excess tissue through a small incision around the edge of the areola.  This could be done under local anesthesia or with intra-venous sedation -- you do not require general anesthesia for this procedure. Removal of the excess tissue would flatten the nipples and the areolar diameter would decrease as well.

You are an ideal candidate for surgical treatment.

i have puffy nippes and was wondering if just tissue removal would flatten them, Also what excuse could I tell people for having the surgery and how long would i be off of school?

( when i squeeze the nipple it self there is nothing there but when i push on it i can feel the tissue)

My nipples look pretty much exactly the same. I have surgery this saturday. My surgeon said he is doing pretty much exactly what Dr. Jacobs said. A incision around the areola then just take some of the gland out.

I most definitely would say yes, you dont look like you have much fat at all.
