It's clear that I have puffy nipples, but they are so big that they have actually begun to sag over the years... would surgery be able to fix the saggy problem or would I just have flat nipples melting down the side of my chest? blah....



Also, I have been 30 lbs heavier than my current weight, as well as 20 lbs lighter... my chest has remained the same though

Sorry, we need better photos.  You look like you are lying down in the photo and everything is distorted by that position.  Why not post some photos of yourself standing up (front and side views)?  Also, please provide info as to your age, weight and maximum weight -- all of that would help to answer your questions.

Would surgery fix it?  Definitely yes -- you have gyne and mild puffy nipples.  No amount of additional weight loss will be of significant help.

Recommend you do your homework and find an experienced gyne surgeon.  In the meantime, you can improve your appearance and posture by wearing a snug compression shirt.
