hematoma progression

I'm almost 7 weeks postop, after having a hematoma on day 2 which was surgically evacuated that same day.

The hematoma side seems to be resolving gradually--I'm trying to be patient. I'm just wondering, at 7 weeks, is the lumpiness that I feel in some areas more likely to be fibrin type clots or scar tissue? My guess is that by now the clots have been dissolved and it's more likely to be scar tissue. Does scar tissue typically shrink from an initial larger size - even without steroid injections?

Palpable lumps after surgery are common with or without a hematoma.  There are areas of induration and swelling, which take time to resolve.  Any associated swelling will also decrease over time, and 7 weeks is still very early in the post-operative course after gynecomastia surgery.  The swelling will continue to decrease over time, and the lumpiness will continue to soften.  I wouldn't address any of your issues with steroid injections at this point, but I do like massage.
