Firmness

I seem to have a firmness around the outer edge of the nipple and not directly under.  Seems
to go all the way around and sometimes causes a puffy effect. Also seems hollow directly under nipple. Is the outer firmness what would be removed?

It's very difficult to answer this without knowing more about your condition or seeing photos--or better yet, examining you.  If there is a mass under the nipple, however, that is most likely what will be excised by the operation.

I would recommend a photo.It is often difficult for a non-surgeon to accurately describe the clinical examination.
