Puffy nipple gyno or overweight?

Hi, i have had these puffy nipples  since i was about 15 and am now 20 so i am getting worried. They have never really been that big but i cant tell if there slowly getting bigger. they sometimes really make me self conscious and I'm honestly sick of it.  I want to know if i just need to lower my bf or look into surgery?
