is there a chance that someone has puffy nipples without having gland inside?

Yes that is possible. When I went to see a breast consultant a  while back he said he believed my nipple was puffed out due to fat rather than gland and he then did an ultrasound to confirm this.

Kind of makes sense as any tissue behind the nipple could puff it out. You do need to be quite overweight for this to happen though as I USED to be ;D

Most cases of Puffy nippes though seem to be caused by Gland .He did say it would be very difficult to get rid of the fat doing this without surgery but not impossible.My nipple does seem slighty smaller since getting fit.

what exactly is ultrasound?

Ultrasound is basically a scan they do to find out what is going on with your chest. They can check for Lumps /fat gland/ etc within your chest.
        It is painless and is very quick . they may call it something different if you are from the states I don't Know.
