Hi, I'm 59 years old, about 5'-8", and weight about 160lbs. I exercise about 5 days a week and am in relatively good shape.  I've lived with gynecomastia most of my life and its always been a source of embarrassment ever since I was a pre-teen. When I was bout 47, I decided to do something about it and had liposuction done on my chest. I was somewhat satisfied with the results until I reached my mid-fifties, that's when I began to notice significant sagging in my chest. So, in September 2020, I decided to have gynecomastia surgery to have the glandular tissue removed. My main objective was not so much to reduce the bulkiness from the chest as it was to reduce the amount of sagging skin along the bottom of my chest. The Dr. who performed my surgery assured my that my skin would contract enough to give me the look I was looking for, but it didn't

As you see in the attached photos, I have excess folding skin along the bottom sides and front of my chest. This is what I am looking to have removed and/or smoothed out as much as possible with minimal scarring. I had  consultation with a Dr. for revision surgery who suggested a pedicle lift without excision. I was hoping for minimal scarring but from I see in photos of other pedicle procedures, these procedures generally result in scarring along the bottom crease of the chest. What I hoping to find out from a Dr. if there's another procedure available where I can remove the excess skin leaving me with a smooth/flat chest (or at least smoother & flatter than it is now with minimal scarring. Your case is very straight forward you need skin excision. Your skin will not contract at your age and currnt condition.  A pedical flap does not solve your issue. The over hang of skin needs removal.  There is no minmal scarring technique to solve your issue.
