I've had minor gynecomastia since puberty and at 21 I've been considering getting the surgery to fix it. My chest is similar to this guy's:

The plastic surgeon I had a consultation with says he's done the surgery many times and has many credentials (although not in gynecomastia surgery), but the way he described it made it seem like all he is concerned about is excising the tissue and not doing much else to shape the nipple or anything. My question is that if I have puffy nipples now and simply just get the tissue removed with this surgeon, is it likely that I'll still have them after the surgery? If so, what would need to be done in addition to the removal of the tissue to ensure a nice completely flat chest? I'm a fairly skinny guy as well, if that means anything.
